The form below must be completed by you, the trip leader, and submitted immediately after holding the Trip Meeting.
The Meeting Sign-in Sheet must be emailed or faxed to your WorldStrides representative. Both are required to receive your reward.
All information is confidential and will not be shared by WorldStrides.
Terms and Conditions: Travel dates must be between 1/1/2017 and 12/31/2018 to qualify. Rewards for invitation distribution and parent meeting attendance will be granted based on the successful completion of the activities outlined above and the submission of the accompanying form no later than June 1, 2017. All monetary rewards are one-time payments that will be mailed within 10 business days of form receipt. WorldStrides complies with federal guidelines for reporting income to teachers. Program Leaders are responsible for knowing and complying with state laws and school policies regarding acceptance, disclosure, and use of financial rewards and prizes. WorldStrides reserves the right to substitute gifts of an equal or greater value.
