This form must be completed by you, the teacher, and submitted immediately after holding the Parent Meeting. The Parent Meeting Sign-in Sheet must be faxed to your WorldStrides representative. Both are required to receive your reward.

Terms and Conditions: Travel dates must be between 1/1/2019 and 7/31/2019 to qualify. Rewards for invitation distribution and parent meeting attendance will be granted based on the successful completion of the activities outlined above and the submission of the accompanying form on WorldStrides’ website no later than December 1, 2018. All monetary rewards are one-time payments that will be mailed within 30 business days of from receipt. WorldStrides complies with federal guidelines for reporting income to teachers. Program Leaders are responsible for knowing and complying with state laws and school policies regarding acceptance, disclosure, and use of financial rewards and prizes. WorldStrides reserves the right to substitute gifts of an equal or greater value.
